Weld County Health Department launches 2025 Community Health Survey

The 2025 Weld Community Health Survey — the most comprehensive source of health information on Weld County residents and one of the most robust county-level health surveys in Colorado — is set to launch on April 1, with invitations going out to approximately 20,000 randomly selected households across the county.
Responses to the survey will help public health experts, municipal governments, health care providers, and local nonprofits better understand residents' needs and helps shape local services and health policies.
“The Weld Community Health Survey is more than just data — it’s the voice of our residents,” said Weld County Commissioner Scott James. “It helps us, and our partners, make smarter, more targeted decisions to help improve public health and is a cornerstone of Weld County’s ongoing effort to create healthy, safe, and livable communities.”
This year’s survey will also explore topics including loneliness, housing costs, and access to physical and mental health care—pressing issues facing many communities.
“This survey is our best tool for understanding the health concerns and needs of Weld County residents,” said Olivia Egen, Public Health Initiatives Manager for the Weld County Department of Public Health and Environment. “We really cannot do this without the generous help of residents. We encourage everyone who receives an invitation to please participate.”
