AAA Internal Services

Weld County AAA offers a wide range of internal services to help high-risk older adults live with independence and dignity in their homes and communities. To aid in this mission, there are five core services provided by Weld County AAA: Caregivers, Elder Rights, Health and Wellness, Nutrition, and Supportive Services. Weld County AAA contracts with community partners or directly administers the programs to deliver these core services. The internal services offered by Weld County AAA are listed below and can also be found in the main resource guide.

Aging and Disability Resources for Colorado (ADRC)/Information and Assistance

The Information and Assistance program is a link to community services, both public and private, for older adults and their families who are interested in becoming aware of the available resources in Weld County.

The Aging and Disability Resources for Colorado (ADRC) program is a single point of entry to assist people age 60 and over, or age 18 and older with a disability, who need an Options Counselor to guide them through a wide range of in-home, community-based and institutional services and programs. The goal of Options Counseling is to provide an individual with all the information needed for them to make an informed decision about long-term care.

Individuals in a nursing facility who receive Health First Colorado and want to transition out of the facility may also receive Options Counseling. Referrals to receive options counseling for transitions may come from the individual, family members, facility staff, or any community member. For more information or assistance, please contact (970) 400-6952 or email AAAinfo@weld.gov.

Adult Protective Services

Adult Protective Services (APS) receives reports of abuse, neglect and exploitation of at-risk adults, conducts an investigation into the allegations, and implements services to reduce and prevent further mistreatment. An at-risk adult is a person 18 years of age or older who is unable to provide or obtain the services necessary for their health, safety and welfare OR who lacks the capacity to make or understand responsible decisions.

Any concerned individual can make a report to APS by calling our intake screeners at (970) 400-6700 or submitting a report online by visiting the Weld County Adult Protection Referral Form. Each report will be evaluated to determine the appropriate response. APS also provides community education regarding abuse/neglect of at-risk adults and the services provided.

Please contact (970) 400-6122 to arrange a presentation.

APS mandatory reporters must call law enforcement dispatch at (970) 350-9600 option 4 to make an initial report of physical abuse, caretaker neglect and/or financial exploitation. Mandatory reporters must call reports in on adults 70 years of age or older or adults 18 years of age or older with an intellectual or developmental disability.

Case Management Agency

Case Management Agency (CMA) provides functional assessments and ongoing case management for qualifying individuals of all ages who are in need of long-term care services in their own homes, alternative care facilities, or for people in need of placement in a skilled nursing facility. In conjunction with the individual, the case manager creates a support plan and assists to arrange services. The case manager provides follow-up monitoring and links individuals and families to community resources.

Weld County residents in need of long-term care services must first apply for Medicaid through Weld County Department of Human Services or online through Colorado PEAK at www.colorado.gov/PEAK. The CMA’s goal is to provide long-term care services and supports in a setting requested by individuals and their families while utilizing Medicaid in the most cost-effective manner.

If you or someone you know is in need of long-term care services or interested in the state general fund programs and may need more information or would like to complete an application, please call the ADRC program at (970) 400-6952. If you are already working with the CMA or would like to contact the CMA, please call (970) 400-6950, option 1.

We can help you answer these and other vital questions.

Support for your family member can be provided in their home, your home, a facility, or in a supportive group setting in the community. Among the choices are the following:

• In-Home Respite Care: Companionship, supervision and assistance for older adults in their home.

• Out-Home Respite Care: Overnight stays in a community facility.

Weld County family caregivers providing informal in-home care to an older adult (60 years or older) who is determined to be “frail,” or family caregivers providing care for individuals with Alzheimer’s Disease and related disorders with neurological and organic brain dysfunction, are eligible to apply.
