HCP is a program for children and youth with special health care needs, serving as a resource for families and health care providers in Weld County. It supports children from birth to age 21 who have, or are at risk for, chronic physical, developmental, behavioral, or emotional conditions requiring more specialized care and services than typically needed.
Nurses and social workers connect families with the information and resources they need to manage their child’s health care.
The program’s goal is to empower families with the knowledge and skills needed to access services and support within the community, enabling them to coordinate their child's care with greater independence and confidence. HCP is dedicated to reducing barriers related to a child’s special health care needs and linking families to vital community programs.
