Public Health Services

The Public Health Services (PHS) division works to make Weld County residents healthier by educating and providing clinical and community health nursing services to support improving individual and public health outcomes. One of our top priorities is to ensure that all community members have access to affordable and quality health care services. By taking a comprehensive, patient-centered approach and collaborating with key community partners, we strive to build a healthier and more resilient community. Housed in the PHS division are the following programs:

Cancer Screening and Prevention

The Women's Wellness Connection (WWC) Program offers free breast and cervical cancer screenings to eligible women in Weld County.

Children with Special Health Care Needs

Supports families with children/youth with special health care needs through in-depth one-on-one care coordination services.

Family Planning

Our Title X Clinic provides individuals the information and means to exercise personal choice in determining the number and spacing of their children.

Immunizations

Provides vaccine services and education about communicable diseases, health risk reduction, and vaccine-preventable diseases to Weld County residents.

Nurse-Family Partnership

A no-cost program that pairs first-time, low-income pregnant women with a specially trained RN who helps them develop healthy nutritional habits, positive relationships, and safe living environments.

Sexually Transmitted Infection Clinic

A full-service clinic for men and women including testing, treatment, and exams for all STIs, including HIV.

Tuberculosis Program

The tuberculosis program provides for evaluation, diagnosis, and treatment for persons with active TB and their contacts.
