Population Health Data and Reports

Providing regular, updated data on the health of Weld County's population is a core function of the Weld County Department of Public Health and Environment. Data are intended to inform the community, including policy makers, medical personnel, researchers, community-based organizations, government agencies, and individuals to understand and respond to diseases, threats, and underlying conditions affecting our health.

Community Health Improvement Plan

The CHIP aims to reduce health disparities and risk for chronic disease and increase years of healthy life among residents.

Community Health Surveys

The Community Health Assessment is conducted every three years to collect local-level data on the health status, behaviors, and concerns and needs of residents.

Healthy Kids Colorado Survey Overview

The Healthy Kids Colorado Survey provides data on risky and health-promoting behaviors among Colorado’s youth. The results can then be used to guide and direct programs and interventions to improve the health of the youth.
