Where to Get a COVID-19 Vaccine

Vaccine Providers

The Colorado Department of Public Health and Environment (CDPHE) has a vaccine provider map, as well as running list of vaccine providers across the state. 

COVID-19 vaccination providers in Weld County include but may not be limited to:

Weld County Health Department

The Weld County Department of Public Health and Environment has the Moderna COVID-19 vaccines available for ages 6 months and up available Monday-Friday from 8:00 a.m. - noon and 1:00-4:30 p.m.

Children must be accompanied by a parent or guardian to their vaccine appointment to provide consent via the Colorado COVID-19 Vaccine Screening and Administration Form (view in Spanish).

Appointments are preferred but walk-ins can be accommodated. To make an appointment to receive the vaccine at the Health Department Clinic, please call (970) 400-6420.  

Dale's Pharmacy

Dale's Pharmacy in Fort Lupton is using an online scheduler to book COVID-19 vaccine appointments for anyone age 5 or older. Moderna and Pfizer vaccines are available.

Good Day Pharmacy

Good Day Pharmacy's Greeley, Eaton, Johnstown, and Longmont locations are offering COVID-19 vaccines.

Kaiser Permanente

Kaiser Permanente members and non-members can make appointments for initial, additional, and booster doses of the COVID-19 vaccine. 

King Soopers

King Soopers Pharmacy is currently vaccinating patients for COVID-19 as vaccine is made available. The Pfizer vaccine is available for youth ages 5-11 and 12-17 at King Soopers. You can make an appointment online to receive the COVID-19 vaccine at a store near you. 

Safeway

Select Safeway pharmacies across the county are offering COVID-19 vaccines. Schedule an appointment online or walk in to get a vaccine of your choice.

Salud

Salud is offering the Pfizer and Moderna COVID-19 vaccines to established Salud patients. Call (303) 697-2583 or visit the patient portal to schedule your vaccine appointment, including for additional or booster doses.  

Sam's Club

Select Sam's Club pharmacies in Colorado are administering COVID-19 vaccines through the U.S. Federal Retail Pharmacy Program. Sam's Club does have Pfizer vaccine available for youth ages 12-17. You do not have to be a Sam's Club member to receive the vaccine at a Sam's Club Pharmacy. Check vaccine locations and availability and schedule an appointment on the Sam's Club Pharmacy website.

Sunrise Community Health

Sunrise is offering COVID-19 vaccines and boosters Monday-Friday on a walk-in basis during regular business hours at these clinics:

You may also reserve a specific time for an appointment by calling (970) 346-2587 (English) or (970) 346-2589 (Spanish).  You do not need to be a Sunrise patient to get a vaccine.  Sunrise offers Pfizer and Moderna vaccines and boosters.

UCHealth

UCHealth has COVID-19 vaccine available for anyone age 12 and older. You do not need to be a UCHealth patient to get a vaccine. You can schedule an appointment online through My Health Connection

Where: Medical Center of the Rockies, 2500 Rocky Mountain Avenue, Loveland, CO 80538. Use the main entrance; clinic is in the Navajo Peak Conference Room.

For individuals who need assistance, they may call the UCHealth COVID-19 Vaccine Hotline at  (720) 462-2255 (English) or (844) 945-2508 (Español). The hotline is open from 7:00 a.m. - 6:00 p.m. Monday - Friday.

Walgreens

Walgreens stores across Weld County are offering Moderna or Pfizer COVID-19 vaccines, depending on the location. Schedule an appointment online at a location near you or walk in. New appointments are made available every 2 hours from 6:00 a.m. - 4:00 p.m. 

Once you schedule your appointment, you'll receive a confirmation and vaccination consent form via email or SMS (if you scheduled over the phone). Bring the completed vaccination consent form to your scheduled appointment (or fill it out in store). A parent or guardian must be present at appointments for all minors. And if you are a myWalgreens member, you also can get $5 Walgreens Cash rewards on $1 purchase in store when you get your free COVID-19 vaccine.

Walmart

Select Walmart pharmacies in Colorado are administering COVID-19 vaccines through the U.S. Federal Retail Pharmacy Program. Walmart does have the Pfizer vaccine available for youth ages 5-11 and 12-17. Check vaccine locations and availability and schedule an appointment online

Pediatric Vaccine Providers

Age-appropriate COVID-19 vaccines are now available for children ages 6 months to 17 years at several locations in Weld County, including but not limited to the following:

Weld County Health Department

The Weld County Department of Public Health and Environment has the age-appropriate Moderna COVID-19 vaccines available for children and adolescents age 6 months to 17 years, Monday-Friday from 8:00 a.m. - noon and 1:00-4:30 p.m.

A parent or guardian must accompany their child to their vaccine appointment in person and provide consent via the Colorado COVID-19 Vaccine Screening and Administration Form (view in Spanish).      

Mobile Vaccination Clinics

The State of Colorado is operating mobile COVID-19 vaccination units with Pfizer and Moderna vaccines as part of Gov. Jared Polis's Vaccines For All initiative. The bus cycles through a number of Weld County locations. You can find upcoming locations online and register for an appointment, but registration is not required. The vaccine is free; insurance and identification are not required to receive the vaccine. Several of these mobile sites also offer the seasonal flu shot.

Daily appointments for the pediatric COVID-19 vaccine are available and can be made via the online scheduler.  

King Soopers

Daily appointments for the pediatric COVID-19 vaccine are available and can be made on the King Soopers website.

Salud

Salud has the pediatric Pfizer COVID-19 vaccine available at its various clinics. The Salud clinics in Commerce City and Fort Lupton are only offering the COVID-19 vaccine to established Salud patients. Call (303) 697-2583 or visit the patient portal to schedule an appointment.

A minor consent form (Spanish) is needed to administer the Pfizer vaccine to any child ages 5 . If you are the parent/guardian and accompanying your child to his/ her vaccine appointment, you can fill out and sign at the clinic. If you are NOT going to be present for your child’s vaccination, please print out and fill out the minor consent form. Salud needs this filled out and signed by the parent and/or guardian to administer the vaccine. 

Sunrise Community Health

Vaccinations for children ages 5-11 will be available at upcoming clinics. Find dates times, and locations, and sign up on the Sunrise website

Walgreens

Walgreens stores across Weld County are offering Moderna or Pfizer COVID-19 vaccines for children 3+, depending on the location. Schedule an appointment online at a location near you or walk in. New appointments are made available every 2 hours from 6:00 a.m. - 4:00 p.m.
