Birth and Death Certificates

The Weld County Department of Public Health and Environment's Vital Records Office provides certified Colorado birth and death certificates to those who meet all requirements.  Our office is open for walk-ins only.  Requests are generally processed the same day.  If the birth or death did not occur in Colorado, please contact the state in which the birth or death occurred.  State Vital Records Offices Contacts(PDF, 92KB)

Birth Certificates

The Weld County Department of Public Health and Environment is able to issue certified copies of birth records for births occurring anywhere in Colorado from 1902 to the present. For births before 1902, the office can issue certified copies only on those births that occurred in Weld County.

Certified copies of birth records are $20.00 for the first copy and $13.00 for each additional copy issued at the same time for the same record.

You also will be required to submit a photo copy of your identification and proof of relationship to purchase birth or death certificates.

Death Certificates

All Colorado death records from 1975 to present are now electronic and the Vital Statistics Office can issue certified copies of these records regardless of the county where the actual death occurred. For deaths earlier than 1975, the office can issue certified copies of those death records only if the death occurred in Weld County.

Certified copies of death records are $20.00 for the first copy and $13.00 for each additional copy issued at the same time for the same record.

Death certificates are now available in three types to serve different purposes:

  • Standard Death Certificate: Lists ALL legal information and ALL medical information for the deceased.
  • Legal Death Certificate: Lists all legal information and NO medical information for the deceased. 
  • Verification of Death: Lists LIMITED legal information and NO medical information for the deceased. This type of certificate is usually recommended for public recordings to protect the deceased’s legal and medical information.

Online 

Customers can submit online requests through GoCertificates, an independent company we have partnered with. An additional fee of $8.00 is charged by GoCertificates for using this service, and all major credit cards are accepted. Orders are processed within 3-5 business days. 

Customers can submit online requests through VitalChek Network Inc., an independent company we have partnered with. An additional fee of $10.00 is charged by VitalChek for using this service, and all major credit cards are accepted. Orders are processed within 3-5 business days.

Customers who mail their application must submit their application with a legible copy of their identification and proof of relationship or tangible interest (for questions on tangible interest please call (970) 400-6430). Forms of acceptable payment are check, money order, Discover®, MasterCard® or Visa®. Orders are processed within 3-5 business days on a first-come, first-served basis. Requests should be mailed to:

Fax

Customers who fax their application must submit their application with a legible copy of their identification and proof of relationship or tangible interest (for questions on tangible interest please call (970) 400-6430. The only forms of acceptable payment are Discover®, MasterCard® or Visa®. Unattended Home Birth Registration

If a birth is not attended by a medically licensed professional, the parent is responsible for registering the birth(PDF, 1MB). If registered within one year of the birth, documentation must be submitted to the local vital records office for the county where the birth occurred.

Correct or Change a Birth Certificate

The state vital records office is the only entity that can make any corrections or changes to a Colorado birth certificate. For all corrections, contact the state vital records office at:
